24-Hour Emergency Assistance

Northeastern Home Group provides 24-hour emergency assistance services for individuals who may require immediate access to support, problem solving, or assistance during health-related or personal emergencies while living in the community. This service is designed to support individuals who do not require 24-hour supervision but may benefit from access to on-call support and immediate response services to help maintain stability and safety within their home environment. 24-hour emergency assistance does not replace emergency services such as 911 during medical or life-threatening emergencies.

Who Qualifies

24-hour emergency assistance may be appropriate for individuals who:

Live in their own home or community-based setting

Do not require 24-hour supervision

Would otherwise require routine or extensive supervision without access to emergency support services

Benefit from access to immediate assistance, problem solving, or on-call support during emergencies

Live with a primary caregiver who may require support accessing assistance during emergency situations

Eligibility is determined through an assessment completed by the county, case manager, or support team based on the individual’s assessed needs and waiver eligibility.

Northeastern Home Group provides Adult Companion services designed to support individuals working toward therapeutic, independent living, and community integration goals within a community-based setting. Adult Companion services provide non-medical support, supervision, and socialization tailored to each person’s individual needs, goals, and level of support. Services are designed to help individuals build independence, increase community involvement, improve daily living skills, and reduce social isolation through structured and person-centered support.

Receive Minnesota HCBS waiver services

Benefit from supervision, socialization, or community support

Need assistance working toward therapeutic or community-based goals

Require support with daily structure and community participation

Night Supervision Services

Northeastern Home Group provides Night Supervision services for individuals who require overnight assistance and supervision within their own home or community-based setting. Night supervision services are designed to support individuals who benefit from overnight staff support related to safety, daily living needs, behavioral support, skill development, or ongoing supervision during nighttime hours. Services are provided by awake or asleep staff based on the individual’s assessed needs and support plan.

Individual Community Living Supports

Northeastern Home Group provides Individual Community Living Supports (ICLS) services for older adults who need reminders, cues, intermittent supervision, or physical assistance to remain safely and independently in their own homes and community settings.

ICLS services are designed to support independence, stability, health, and meaningful community involvement through individualized and person-centered support.

Services are based on the person’s assessed needs, goals, and support plan and are coordinated in collaboration with the individual, family members, case managers, and support teams.

Who Qualifies

Respite services may be appropriate for individuals who:

Are age 65 or older

Are enrolled in the Alternative Care (AC) program or Elderly Waiver (EW)

Need assistance remaining safely in their home or community setting

Benefit from reminders, cues, intermittent supervision, or physical assistance

Require support in at least two ICLS service components identified through assessment and service planning

Eligibility is determined through an assessment completed by the county, case manager, or care coordinator based on the person’s assessed needs and program eligibility

In-Home Respite Care Services
